Qualified Affiliate: I would like to become an USIU's Qualified Affiliate, which will allow me to receive commissions/discount of 20% on my personal sales. I will also be able to sponsor other affiliates. I can receive additional residual income of 10% of the tuition cost of my personally sponsored affiliates. I will be given a personal internet link direct to the USIU's website. This will allow my clients worldwide, to make purchases or enroll into any of the educational programs or services through me, twenty-four hours a day. I receive a scholarship of $500.00 to use for myself, to give as a gift or sell to others. The cost of starting my own business as Qualified Affiliate and getting on the fast tract to success is only $499.99.

Affiliate Associate: I would like to become an USIU's Affiliate Associate, which will allow me to receive commissions/discount of 10% on my personal sales. I will also be able to sponsor other affiliates. I will not receive residual income from my personally sponsored affiliates until I have been certified as a Qualified Affiliate. Residual income does not apply to sponsorships prior to date of certification and is not retroactive. Upon paying $499.99 or having sales of $1500.00 or more, you can then become a Qualified Affiliate. I will be given a personal internet link direct to the USIU's website. This will allow my clients worldwide, to make purchases or enroll into any of the educational programs or services through me, twenty-four hours a day. I receive a scholarship of $200.00 to use for myself, to give as a gift or sell to others. The cost of starting my own business as an Affiliate Associate and getting on the fast tract to success is only $249.99.

Interim Affiliate: Yes, I want to start my own business as an Interim Affiliate (IA) which will allow me to distribute USIU's Educational products and services. I will receive a 5% commission/discount on my personal sales or purchases. I will not be able to sponsor others and will not receive residual income until I have been certified as a Qualified Affiliate. Residual income does not apply to sponsorships prior to date of certification and is not retroactive. Upon paying $499.99 or having sales of $1700.00 or more, you can then become a Qualified Affiliate. I will be given a personal internet link direct to the USIU's website. The cost of registering as Interim Affiliate is only $99.99.

INDEPENDENT AFFILIATE AGREEMENT

By placing my signature below, I acknowledge that I am eighteen years or older and that I have carefully read and understand all of the terms and conditions set forth in this distributorship application and agree to abide by them. I understand that none of our products or services are to be sold on Ebay or similar programs . I also understand that USIU's policies and procedures are incorporated by reference into this agreement and that all unresolved disputes are to be resolved by arbitration at the place of USIU's choice, and that this distributor agreement is in effect for one full year from its date of acceptance.
